How to file a complaint regarding healthcare services and related issues in the District:

Department of Health - If you have experienced what you believe to be unfair, inadequate or unsafe treatment at a healthcare facility, including: nursing homes; home health agencies; end-stage renal disease facilities; laboratories; and intermediate care facilities please call DOH's Health Care Facilities Division at (202) 442-5888 or submit an incident report online.

Department of Mental Health - If you would like to speak to someone regarding an incident with a private mental health facility in the District, call DMH's Mental Health Authority at (202) 673-4374.  For questions or concerns about services provided at a DMH facility, call the DMH Community Service Agency at (202) 444-4100.

Private Insurance - The Department of Insurance, Securities and Banking handles private health insurance matters. To file a complaint, submit the online insurance complaint form.

Environmental Health - You can call these centers if you need to report an incident regarding lead poisoning, pharmaceutical control, food inspection/protection, rodent control or other environmental health hazards.
